Prescription Refills

Please request your medication refills during normal business hours. In the event you need a refill after office hours, you may be charged a $10.00 fee. Please plan ahead before your medication is due to run out.

Insurance & Referrals

It is the patient's responsibility to know if your insurance requires a referral. Please contact your primary care physician prior to your appointment if a referral is necessary.

Fees & Payments

Every effort is made to keep our fees reasonable. Fees for professional services are based on the amount of physician time involved and the degree of complexity of the patient's problem. Fees for laboratory, EKG's, pulmonary function procedures, flexible sigmoidoscopies and other diagnostic testing are charged separately from the physician's professional service fee and are itemized on your account. Payment for services is set forth in our written Credit Policy. If you have not received or reviewed our Credit Policy, please feel free to request a copy from our Business Office or Receptionist. All co-pays are due at time of service. We accept check, cash, money orders, Visa, MasterCard and Discover credit cards.
